Job summary

Pacific Health Group seeks an ECM Program Manager to lead our Enhanced Care Management program across California. You will supervise Lead Care Managers, ensure high-quality in-person and coordination services, and drive outcomes for high-need populations.

This role emphasizes leadership, budget oversight, program development, and cross-functional partnerships with hospitals and community organizations. Regular travel within the hiring counties and occasional after-hours work are expected.

Qualifications

• Leadership in healthcare settings or care management programs.
• Strong knowledge of care/case management principles and Medicaid/Medicare ECM requirements.
• Ability to analyze data to drive program improvements.

Responsibilities

• Lead and mentor Lead Care Managers across multiple counties.
• Oversee program development, quality assurance, and workflows.
• Monitor member services and ensure high-quality, in-person care.
• Collaborate with hospitals, providers and community organizations.
