Job summary

Correctional Health Treatment Centers, Inc. is seeking a Lead Care Manager to oversee and coordinate ECM care management, ensuring our clients receive optimal support and resources. This role requires effective communication, organizational excellence, and a strong commitment to client-centered care.

The Lead Care Manager will lead assessments, develop Care Plans, and coordinate services with providers and the ECM care team to close gaps in treatment and services.

Qualifications

• Bachelor’s degree in Social Work, Nursing, or related field preferred, but not required.
• Experience in care management or healthcare setting preferred but not required.
• Strong understanding of healthcare regulations, policies and best practices.
• Experience in case management and interacting with clients, including SDoH concerns.
• Excellent communication, interpersonal and problem-solving skills.
• Proficiency in care management software and electronic medical records, Microsoft Office and similar software.

Responsibilities

• Serve as the primary point of contact for ECM program clients and care team.
• Develop comprehensive Care Plans with input from client and care team.
• Coordinate services from various providers for ECM program participants.
• Provide case management and liaison services per each client’s plan.
• Develop SMART goals, implement and review progress, revise as needed.
• Maintain communication with ECM Care Team and other agencies.
