Job summary

Pacific Health Group is seeking an ECM Program Manager to lead the Enhanced Care Management program across California, guiding regional teams to deliver person-centered care that improves outcomes and advances health equity.

You will oversee program development, quality assurance, budgeting, and cross-functional collaboration with hospitals, providers, and community organizations, while driving growth and ensuring regulatory compliance.

Qualifications

• Bachelor’s or Master’s degree in Healthcare Administration, Public Health, Nursing, or related field.
• 5+ years in healthcare management, including 3+ years in care/case management.
• 2+ years managing Enhanced Care Management or intensive care management programs.

Responsibilities

• Lead Lead Care Managers across multiple counties and conduct regular coaching.
• Develop and implement program quality assurance protocols and workflows.
• Oversee member services, ensure in-person visits, and monitor care documentation.
