Job summary

Paylocity is seeking a lead-level resource to oversee Enhanced Care Management (ECM) programs, ensuring high-quality services and compliance with contractual and regulatory requirements.

You will supervise staff, oversee day-to-day operations, and monitor performance to drive optimal member outcomes. The role requires collaboration with providers, community partners, and internal stakeholders, plus travel within California and a valid CA driver’s license.

Qualifications

• Minimum 2 years of direct service experience with children, families, or vulnerable populations.
• 2+ years of case management or care coordination experience.
• Bilingual in English and Spanish or English and Hmong.
• Experience with ECM, CalAIM, Medi-Cal Managed Care or related health and human services programs.
• Proficiency with data systems and Microsoft Office applications.

Responsibilities

• Oversee day-to-day operations of assigned ECM programs and ensure services meet goals, funding and regulatory requirements.
• Assist with staff supervision, training, coaching, and mentoring.
• Serve as primary contact for programs and coordinate staffing and service delivery.
• Monitor operations and service quality, ensure compliance with policies and regulations.
• Collect, analyze, summarize, and report program data and outcomes for funding sources and leadership.
• Maintain documentation and ensure confidentiality and HIPAA compliance.
