Job summary

Sacramento Children's Home is seeking a Care Manager Lead to oversee the Enhanced Care Management (ECM) program in Sacramento. The role balances leadership with direct service, supervising a team and managing an active ECM caseload of about 25 members.

You will ensure fidelity to CalAIM ECM, coordinate medical, behavioral health, housing, and social resources, and participate in quality improvement and training activities. A strong background in care coordination and supervision is essential.

Qualifications

• High school diploma or GED required.
• Master's or Bachelor's in Social Work or related field preferred.
• Active California licensure preferred; LCSW/LMFT/LPCC/RN applicable where needed.
• Three years of care coordination or behavioral health experience preferred.
• Two years of supervisory or team leadership experience preferred.
• Experience with Medi-Cal, CalAIM ECM, or integrated care programs preferred.

Responsibilities

• Provide leadership, supervision and program oversight for the ECM team.
• Maintain an active ECM caseload of ~25 members and supervise care managers.
• Oversee documentation, billing compliance and quality indicators.
• Coordinate interdisciplinary care and community/resource referrals.
• Contribute to training, onboarding, and program improvement initiatives.
• Participate in after-hours crisis response as assigned.
