Job summary

ArchWell Health in Tucson, AZ is seeking a Referral Coordinator to guide members through the care system, coordinating internal and external referrals and ensuring timely appointments. You will support seniors with care navigation and documentation while collaborating with a multidisciplinary team.

The ideal candidate has experience in referral management, knowledge of Medicare criteria, strong communication skills, and Spanish language ability where needed; you will help senior members access

Qualifications

• 2 years of experience in referral management or care coordination.
• Experience with EMR systems, ideally eClinicalWorks or similar.
• Ability to guide members and families through care options.
• Respect patient confidentiality and HIPAA standards.
• Strong communication with diverse populations.
• Spanish fluency or needed language skills where applicable.

Responsibilities

• Coordinate referrals to external providers, vendors, and services.
• Document referrals accurately to ensure records are current.
• Support members navigating the healthcare system and care plans.
• Build relationships with members and families to facilitate care coordination.
• Monitor progress and follow up on care plan goals.
• Adhere to HIPAA standards and confidentiality of PHI.
• Assist with problem solving barriers to care (translation, transportation, etc.).
• Serve as system navigator and main contact for members and families.
