The ideal candidate has 2+ years in referral management or care coordination, familiarity with Medicare criteria, and proficiency with EMR systems such as eClinicalWorks.

Qualifications

• Deep understanding of the healthcare ecosystem, including benefits and insurance authorization.
• Ability to work effectively with diverse populations and an interdisciplinary care team.
• Fluency in Spanish or other community languages where applicable.
• Knowledge of Medicare criteria for equipment and supplies.
• 2 years of referral management or care coordination experience.

Responsibilities

• Partner with ArchWell Health's care team to understand member care needs and gaps in treatment.
• Coordinate referrals to external providers and services to meet care needs.
• Document referrals to keep records accurate and current.
• Build relationships with members and families to facilitate care coordination.
• Monitor care plan progress and ensure follow-up actions are completed.
• Adhere to HIPAA and protect PHI.
• Promote clear communication among care team and clinicians.
