Job summary

ArchWell Health in Phoenix seeks a Referral Coordinator to guide members through the healthcare system, ensuring referrals to the right specialists and settings are timely and appropriate. Qualifications

• Deep understanding of the healthcare ecosystem and how referrals flow.
• Experience with benefits and insurance authorization processes.
• Medicare criteria for equipment and supplies knowledge is helpful.
• EMR experience and familiarity with care coordination workflows.

Responsibilities

• Partner with care team to understand each member’s needs and identify gaps.
• Coordinate referrals to external providers to meet care plans.
• Document member referrals accurately in the EMR.
• Build relationships with members and families to facilitate care.
• Monitor progress on care plan goals and follow up.
• Protect PHI and adhere to HIPAA standards.
• Facilitate clear communication among clinicians to support outcomes.
• Assist with problem solving related to health system, transportation, or financial barriers.
