Job summary

ArchWell Health is seeking a Referral Coordinator in Phoenix to help members navigate the healthcare system and ensure they receive the right care in the right setting. This role partners with a multidisciplinary team to coordinate referrals and document processes, while protecting PHI and communicating clearly with members and families.

The ideal candidate has 2 years in referral management or care coordination, a HS diploma, and EMR experience (eClinicalWorks preferred).

Qualifications

• Minimum 2 years in referral management or care coordination.
• HS Diploma or equivalent required; healthcare coursework preferred.
• Experience with EMR systems, ideally eClinicalWorks or similar.

Responsibilities

• Partner with care team to understand member care needs and identify gaps.
• Coordinate referrals to external providers to meet care plans.
• Document referrals accurately and maintain current records.
• Build relationships with members and families to facilitate care coordination.
• Ensure HIPAA compliance and protect PHI; promote clear team communication.
