Job summary

ArchWell Health in Phoenix, AZ is seeking a Referral Coordinator to help members navigate the healthcare system, ensuring they meet with the right specialists in a timely manner. The ideal candidate will understand Medicare criteria, CPT/ICD-10 codes, EMR systems such as eClinicalWorks, and communicate effectively with diverse populations, patients, families, and providers.

Qualifications

H.S. Diploma or equivalent; healthcare coursework preferred
2 years in referral management or care coordination
Medical Assistant or medical administrative experience helpful
EMR experience, ideally eClinicalWorks or similar

Responsibilities

Partner with care team to understand member care needs and identify gaps
Coordinate referrals to external providers, vendors, and services to meet care needs
Document referrals accurately and keep records current
Build relationships with members and families to facilitate care coordination
Monitor progress on care plan goals and follow up as needed
Adhere to HIPAA standards and protect PHI
Promote clear communication among care team and clinicians
Assist members with problem solving related to barriers in care (transport, interpreters, etc.)
Serve as system navigator and primary contact for members and families
