Job summary

SCAN Group in Riverside, CA is seeking a Community Health Worker to support older adults by conducting in-home visits, assessing needs, coordinating with the care team, and helping patients access Medi-Cal, SNP, housing assistance, and transportation.

You will advocate for patients, document plans and progress, and participate in training while traveling about 50% of the time in Riverside and surrounding areas.

Qualifications

• High school diploma or equivalent required; associate degree preferred.
• Community Health Worker certificate preferred.
• Training or education in social work or benefits navigation preferred.
• Travel 50+% of the time in the assigned area with valid driver's license and reliable transportation.
• 2-3 years of related experience in managed care, healthcare, or case management.
• 1+ year working with seniors, conducting home visits, and remote work strongly preferred.
• Bilingual Spanish proficiency preferred but not required.

Responsibilities

• Conducts in-home or telephonic visits to assess the patient and home environment and identify barriers to optimal care.
• Develops and implements plans to address identified barriers with patient, family, and care team.
• Advocates for patients to access healthcare and assistance, including Medi-Cal, SNP, subsidies, housing, transportation, and caregiving.
• Supports care planning conversations and educates patients on chronic disease management and health system navigation.
• Coordinates primary or specialty care, labs, imaging, and transportation to appointments.
• Maintains privacy and confidentiality of patient information across systems.
• Completes timely documentation of assessments, plans, and progress notes in multiple systems.
• Participates in educational offerings and team case conferences per department guidelines.
• Utilizes community resources and identifies barriers to work processes with supervision as needed.
• Demonstrates strong communication, teamwork, and problem-solving to achieve outcomes.
• Acts as a liaison with community agencies to support member needs.
