Job summary

SCAN Health Insurance is seeking a Patient Care Coordinator to support SCAN clinicians and coordinate comprehensive care for members, helping seniors stay healthy and independent in the Riverside area.

You will process orders, arrange services, coordinate appointments and transportation, collect reports, and communicate with members, caregivers, clinicians, and vendors. This remote/field role requires strong organization and compassionate, solutions-focused service.

Qualifications

• High School Diploma or equivalent required.
• 1+ years of related experience required.
• Medical Assistant certification preferred.
• Bilingual Spanish proficiency preferred; not required and will be tested if applicable.

Responsibilities

• Coordinate and deliver healthcare by working with SCAN clinicians.
• Process orders, relay to vendors, arrange services, and obtain reports.
• Coordinate patient appointments and transportation; ensure insurance acceptance and documentation.
• Obtain patient care records from hospitals, doctors, labs, and vendors.
• Manage calls, mail, faxes, and emails; prepare forms and refill prescriptions.
• Maintain inventory of supplies for clinical staff and stock as needed.
• Conduct home visits to complete assessments or deliver supplies.
• Communicate professionally with members, caregivers, clinicians, vendors, and partners.
