Job summary

SCAN Group is a nonprofit health organization seeking a Community Care Specialist to support frail seniors, assess needs, and coordinate care remotely and in the field across the San Bernardino area. You will work with the care team to implement plans of care, advocate for members, and enable independence.

The role emphasizes bilingual communication, field visits, and collaboration with healthcare providers. A strong commitment to senior care, confidentiality, and community resources is required.

Qualifications

• Bilingual English/Spanish required; proficiency assessed.
• Travel 50%+ in assigned area; valid driver's license required.
• 1+ year of Community Engagement experience preferred.
• Experience with managed care or healthcare settings is preferred.
• 1+ year working with seniors, conducting home visits, and remote work preferred.

Responsibilities

• Conduct assessments to identify chronic conditions and psychosocial challenges affecting independent living.
• Support member well-being through observation, communication, and coaching.
• Provide care coordination, education, navigation, and referrals to community resources.
• Perform ECM activities for specified Populations of Focus.
• Develop personalized care plans and ensure interdisciplinary planning.
