SCAN Health Plan seeks a Patient Care Coordinator to support clinicians and coordinate care for members across Riverside, CA and surrounding areas. The role involves processing orders, coordinating appointments and transportation, and obtaining reports to ensure timely service delivery.

Responsibilities include handling calls, refilling prescriptions, maintaining inventory, and conducting home visits or virtual visits as needed.

Qualifications

• High School Diploma or equivalent experience.
• Associate's degree or equivalent experience preferred.

Responsabilités

• Coordinate and deliver excellent healthcare to SCAN members.
• Coordinate patient appointments and transportation, ensuring insurance acceptance and required documentation.
• Obtain patient care records and results from hospitals, doctor’s offices, labs and other vendors.
• Manage and triage incoming calls, mail, faxes and emails, including preparing forms and refilling approved prescriptions.
• Conduct home visits with patients to complete assessments, deliver essential supplies, or facilitate virtual clinical visits.
• Communicate professionally with members, caregivers, clinicians, vendors, and community partners while promoting SCAN’s mission.
