SCAN is seeking a Patient Care Coordinator to support clinicians and coordinate care for SCAN members to help keep seniors healthy and independent. The role blends remote work with field visits in Riverside, CA, about 25% of the time, and involves coordinating appointments, orders, and transportation while maintaining documentation and vendor relations.

Responsibilities include working with clinicians to deliver care, managing calls and correspondence, and ensuring timely service delivery.

Qualifications

• High School Diploma or equivalent experience.
• Associate's degree or equivalent experience preferred.
• 1+ years of related experience required.
• Training or certification as a Medical Assistant preferred
• Bilingual Spanish proficiency is preferred but not required.

Responsabilités

• Work closely with SCAN clinicians to coordinate and deliver healthcare to members.
• Independently coordinate essential patient care by processing orders, relaying orders to vendors, arranging services, ensuring timely service delivery, and obtaining reports or results when applicable.
• Coordinate patient appointments and transportation, ensuring specialists accept patient insurance and have all necessary documentation or authorization, visits are conducted on time, and that we obtain documentation and recommendations from visit.
• Obtain patient care records and results from hospitals, doctor’s offices, labs and other vendors.
