Job summary

SCAN is seeking a Community Engagement Specialist to enhance frail seniors’ ability to age in place by telephonically and in-person care coordination. You will assess needs, advocate for members, and implement plans of care in alignment with ECM requirements.

The role involves field visits in the San Bernardino area, collaboration with care teams, and direct engagement with members to support primary and preventive care, with a strong emphasis on independence and quality of life.

Qualifications

• High School Diploma or equivalent required or 2+ years of related experience.
• Community Health Worker Certificate strongly preferred.
• Bilingual English/Spanish required; a proficiency test may be administered.

Responsibilities

• Perform general assessments to identify chronic conditions and psychosocial challenges in independent living.
• Conduct Medi-Cal/Medicare dual-eligible assessments for LTSS referrals.
• Provide care coordination, coaching, and navigation to community resources.
• Coordinate Enhanced Care Management activities for specified populations of focus.
• Implement personalized care plans and participate in interdisciplinary planning for smooth transitions.
