Qualifications

• High School Diploma or equivalent experience.
• Community Health Worker Certificate preferred.
• Training or education in social work or benefits navigation preferred.
• Travel 50+% of the time in the assigned area; valid driver’s license and reliable transportation.
• 2-3 years of related experience in managed care, healthcare environment, lived experience or case management.
• 1+ year working with seniors, home visits, and remote work preferred.
• Bilingual Spanish proficiency preferred but not required; assessment may be conducted.

Responsibilities

• Conducts in-home or telephonic visits to assess patients and home environment.
• Develops and implements a plan to address barriers with patient and care team.
• Advocates for patients to obtain healthcare and assistance, including MediCal or SNP, subsidies, housing, or transportation.
• Supports goals-of-care conversations and educates on chronic disease management and system navigation.
• Coordinates primary or specialty care, labs, imaging, and transportation.
