Job summary

SCAN Health Plan seeks a Patient Care Coordinator to support clinicians and coordinate care for SCAN members, helping keep seniors healthy and independent. You will work closely with clinicians, coordinate orders, arrange services, and obtain reports while ensuring timely delivery and documentation.

Field work will occur in Riverside, CA with remote capabilities. The role requires a High School Diploma, 1+ year of related experience, and preferred Medical Assistant certification; bilingual

Qualifications

• High School Diploma or equivalent experience.
• Associate's degree or equivalent experience preferred.
• 1+ years of related experience required.
• Training or certification as a Medical Assistant preferred.
• Bilingual Spanish proficiency is preferred but not required.

Responsibilities

• Coordinate care by working with SCAN clinicians to deliver healthcare to members.
• Process orders, relay orders to vendors, arrange services, and obtain reports or results.
• Coordinate patient appointments and transportation ensuring insurance acceptance and required documentation.
• Obtain patient care records from hospitals, clinics, labs and vendors.
• Manage calls, mail, faxes and emails and refill approved prescriptions.
• Maintain inventory and stock of supplies for clinical staff.
• Conduct home visits to complete assessments or deliver supplies and facilitate virtual visits.
