Job summary

SCAN Health Plan in Riverside, CA is seeking a Patient Care Coordinator to support clinicians and coordinate care for members, ensuring timely services and proper documentation. This role combines remote work with field visits about 25% of the time.

You will coordinate orders, arrange transportation, obtain records, and triage communications while maintaining compliance and high-quality care across our community of seniors.

Qualifications

• High School Diploma or equivalent.
• Associate's degree preferred; related certifications optional.
• 1+ years of related experience required.
• Medical Assistant training or certification preferred.
• Performs work under general supervision.
• Handles moderately complex issues and refers to higher-level staff.
• Bilingual Spanish proficiency preferred.

Responsibilities

• Coordinate and deliver patient care by processing orders and relaying to vendors.
• Arrange services, ensure timely delivery, and obtain reports or results.
• Coordinate patient appointments and transportation with insurance acceptance.
• Obtain patient care records from hospitals and clinics.
• Manage calls, mail, faxes, and emails; prepare forms and refill prescriptions.
• Maintain inventory and stock supplies for clinical staff.
• Conduct home visits to complete assessments or deliver supplies.
• Communicate professionally with members, caregivers, clinicians, and partners.
• Identify improvements from patient feedback and propose solutions.
• Support SCAN’s AI initiatives and vision.
