Job summary

SCAN Health Plan in Long Beach, CA seeks a Manager Care Coordination to lead teams delivering education and assistance to members, navigate healthcare systems, and ensure regulatory compliance.

You will oversee day-to-day operations, recruit and train staff, develop metrics, and partner with stakeholders to improve outcomes while maintaining high standards across CMS/NCQA requirements. This is a mostly remote, full-time role with an annual bonus and strong benefits.

Qualifications

• Bachelor's Degree or equivalent experience.
• 5+ years in healthcare, medical management, change management or leadership.
• Working knowledge of NCQA standards, SNP requirements and CMS/DHS regulations.
• Able to lead without direct reporting relationships and influence outcomes.
• Strong written and verbal communication skills.

Responsibilities

• Lead teams through day-to-day operations to meet department goals.
• Oversee workflows, priorities and projects within timeframes.
• Recruit, orient and train staff; maintain a safe work environment.
• Develop and review department metrics and reporting.
• Support supervisors with escalated issues and policy enforcement.
• Partner with stakeholders to align with organizational goals.
