SCAN Health Insurance seeks a field/remote Care Manager to enhance frail seniors’ ability to age in place and navigate the health system. You will work telephonically and in-person to implement plans of care, advocate for members, and provide member-centered interventions.

You will perform assessments, support primary and preventive care, and coordinate services with a multidisciplinary team, while ensuring timely documentation and data integrity.

Qualifications

• High School Diploma or equivalent experience.
• Community Health Worker Certificate preferred.
• Bilingual English/Spanish required; testing may apply.
• Travel 50%+ within the assigned area with a valid driver’s license.

Responsabilités

• Perform assessments to identify chronic conditions and psychosocial needs.
• Conduct in-person or phone visits to support care plans.
• Coordinate care and navigate healthcare for members and families.
• Carry out Enhanced Care Management activities for specified populations.
• Collaborate in interdisciplinary planning and document plans and progress.
