Job summary

SCAN Health Plan, a nonprofit health organization serving seniors, seeks a Patient Care Coordinator to support clinicians and coordinate care for members. You will process orders, relay to vendors, arrange services, and obtain reports when applicable.

You will also schedule appointments, arrange transportation, obtain documentation, and triage communications while maintaining inventory and aiding home visits as needed.

Qualifications

• High School Diploma or equivalent experience.
• Associate's degree or equivalent experience preferred.
• 1+ years of related experience required.
• Training or certification as a Medical Assistant preferred.
• Bilingual Spanish proficiency preferred but not required.
• Handles moderately complex issues with guidance.

Responsibilities

• Coordinate care for SCAN members by working with clinicians.
• Process orders, relay to vendors, arrange services, and obtain results.
• Schedule appointments and transportation ensuring insurance coverage.
• Obtain patient care records from hospitals, doctors, labs, and vendors.
• Manage calls, mail, faxes and emails; refill approved prescriptions.
• Maintain inventory and stock supplies for clinical staff.
• Conduct home visits to complete assessments or facilitate virtual visits.
• Communicate professionally with members, caregivers, clinicians and partners.
• Identify improvements from patient feedback and propose solutions.
• Support SCAN's vision and goals; perform other duties as assigned.
