ArchWell Health is seeking a Care Navigator to provide a warm, welcoming experience for our senior members and their families at our neighborhood clinics. You will guide members from arrival through departure, ensuring their care needs are met with exceptional service.

The ideal candidate will have a strong customer-service orientation, passion for senior care, and the ability to work with diverse populations.

Qualifications

• Associates degree preferred, or equivalent experience.
• Minimum of one year of work experience in a clinical setting or similar.
• Experience working with EMR systems is a plus.

Responsibilities

• Welcome members upon arrival and guide them through check-in.
• Update member information in the electronic medical records system.
• Schedule appointments and coordinate referrals for a multi-disciplinary care team.
• Assist members with filling out paperwork through electronic kiosks.
• Request medical records and upload documents to EMR system.
• Field questions from members and their families.
• Manage phone line and relay details to the right team.
• Assist with center events and activities as needed.
