ArchWell Health seeks a Care Navigator to provide a positive, welcoming experience for members and families from arrival through departure. The role focuses on guiding members, coordinating care, and ensuring their needs are met with courtesy and efficiency.

The ideal candidate will have a strong customer-service orientation, passion for senior care, and the ability to work with diverse populations including staff, providers, members, and families. Experience with EMR systems is a plus.

Qualifications

• Excellent customer service with a welcoming demeanor.
• Passion for senior care and helping members and families.
• Ability to work with diverse populations and teams.
• Working knowledge of medical terminology and EMR systems.
• Strong organizational skills and attention to detail.
• Clear written and verbal communication.
• Proficient in MS Office and general PC use.
• Maintain professionalism and flexibility in changing environments.
• Fluency in Spanish or other languages used by the communities served.

Responsibilities

• Welcome members and guide them through check-in to ensure a positive, home-like experience.
• Update member information in the EMR system.
• Schedule appointments and coordinate referrals for a multi-disciplinary care team.
• Assist members with filling out paperwork via electronic kiosks.
• Request medical records and upload documentation to EMR.
• Field questions from members and their adult children.
• Manage the front desk phone line and relay messages to the appropriate team.
• Assist with center events as needed.
