Job summary

ArchWell Health in Overland Park, KS is seeking a Care Navigator to warmly welcome members from arrival to departure and ensure their care needs are met. You will guide seniors and families through check-in, scheduling, referrals, and paperwork, while updating records in our EMR system.

Ideal candidates have strong customer service, attention to detail, and the ability to communicate clearly with diverse populations. Fluency in Spanish or other community languages is a plus.

Qualifications

• Associates degree preferred or equivalent experience.
• Minimum of one year in a clinical setting.
• Experience working with EMR systems is a plus.

Responsibilities

• Welcome members upon arrival and create a welcoming atmosphere.
• Update member information in the EMR system.
• Schedule appointments and coordinate referrals for a care team.
• Assist with filling out paperwork via electronic kiosks.
• Request medical records and upload documentation to EMR.
• Address questions from members and families.
• Manage center phone line and relay information to appropriate staff.
