Qualifications

• Excellent customer service orientation and welcoming demeanor.
• Ability to work effectively with diverse populations and stakeholders.
• Working knowledge of medical terminology, insurance, and electronic medical records systems.

• Strong written and verbal communication and professionalism.

Responsibilities

• Welcome members and ensure a safe, at-home feel during check-in.
• Update member information in the EMR system.
• Schedule appointments and coordinate referrals for a multi-disciplinary care team.
• Assist members with filling out paperwork through electronic kiosks.
• Request medical records and upload documents to EMR.
• Answer questions from members and their families; relay details to the right staff.
• Manage the center phone line and relay messages accurately.
• Assist with center events as needed.
