ArchWell Health seeks a Care Navigator to welcome members and families at our neighborhood clinics, guiding them from check-in through checkout while ensuring their care needs are met with kindness and efficiency.

The role requires strong customer service, a passion for senior care, and the ability to interact with diverse populations. EMR experience and bilingual abilities are highly valued.

Competenze

• Associates degree preferred, or equivalent experience.
• At least 1 year of work experience in a clinical setting or similar.
• Experience with EMR systems is a plus.

Mansioni

• Welcome members upon arrival and help them feel at home, guiding them through a smooth check-in process.
• Update member information in the electronic medical records system.
• Schedule appointments and coordinate referrals for a multi-disciplinary care team.
• Assist members with filling out paperwork through electronic kiosks as needed.
• Request medical records and upload documentation to the EMR system.
• Field questions from members and their adult children.
• Manage the front desk phone line and relay messages to the appropriate team.
• Assist with center events as needed.
