Job summary

ArchWell Health is seeking a Care Navigator in Overland, MO to welcome members, guide them through check-in, and coordinate care with our multidisciplinary team. You will update EMR records, assist with paperwork, and help families navigate appointments and referrals.

The ideal candidate has a strong customer-service orientation, a passion for senior care, and the ability to communicate effectively with diverse populations. Fluency in Spanish or other languages is a plus.

Qualifications

• Associates degree preferred or equivalent experience.
• Minimum of one year in a clinical setting or similar.
• Experience with EMR systems is a plus.

Responsibilities

• Welcome members and guests and ensure a safe, at-home feel while checking in.
• Update member information in the EMR system.
• Schedule appointments and coordinate referrals for a multi-disciplinary team.
• Assist with completing paperwork electronically.
• Request medical records and upload to EMR.
• Answer questions from members and their families.
• Manage incoming calls and relay messages accurately.
• Assist with center events as needed.
