ArchWell Health is seeking a Care Navigator to welcome members and families, guiding them from arrival to departure with a focus on a positive, seamless experience. The role supports check-in, scheduling, and documentation in EMR systems, working with a multidisciplinary team to ensure care needs are met.

The ideal candidate has strong customer-service orientation, a passion for senior care, and excellent communication with staff, members, and families.

Qualifications

• Excellent customer service and a welcoming demeanor.
• Passion for delivering quality senior care.
• Ability to work with diverse populations including staff, providers, members, families, and vendors.
• Working knowledge of medical terminology, insurance, and EMR systems.
• Strong organizational skills and attention to detail.
• Clear written and verbal communication.
• Proficient PC skills including Microsoft Office.

Responsibilities

• Greet members and guide them through check-in so they feel at home.
• Update member information in the electronic medical records system.
• Schedule appointments and coordinate referrals for a multidisciplinary care team.
• Assist with filling out paperwork via electronic kiosks.
• Request medical records and upload documentation to EMR.
• Answer questions from members and families; relay to appropriate staff.
• Assist with center events as needed.
