ArchWell Health is seeking a Care Navigator to create a welcoming, smooth experience for our senior members from check-in through discharge. You will guide patients, help with paperwork at electronic kiosks, and coordinate referrals within a multidisciplinary care team.

The ideal candidate has a strong customer-service orientation, attention to detail, and experience in a clinical setting with EMR systems; Spanish or other languages spoken in our communities is a plus.

Qualifications

• Associate degree preferred or equivalent experience.
• Minimum of one year of work experience in a clinical setting.
• Experience with EMR systems is a plus.

Responsabilités

• Welcome members upon arrival and help them feel safe and at home, guiding them through check-in.
• Update member information in the EMR system.
• Schedule appointments and coordinate referrals for a multidisciplinary care team.
• Assist members with filling out paperwork through electronic kiosks.
• Request medical records and upload documentation to the EMR.
• Field questions from members and their adult children.
• Manage the phone line, take messages, and relay details to the right team.
• Assist with center events as needed.
