Job summary

ArchWell Health is seeking a Care Navigator to welcome members and their families, guiding them from arrival through check-in, visiting, and post-visit steps with warmth and accuracy. You will help members with paperwork, schedule appointments, coordinate referrals, and maintain EMR notes to ensure smooth care delivery.

Ideal candidates bring exceptional customer service, comfort with medical terminology and office software, fluency in Spanish or other languages used in the communities served,

Qualifications

• Associates degree preferred or equivalent experience.
• Experience with EMR systems is a plus.
• Strong customer service focus and communication skills.

Responsibilities

• Welcome members upon arrival and help them feel safe and at home, guiding them through check-in.
• Update member information in the electronic medical records system.
• Schedule appointments and coordinate referrals for a multi-disciplinary care team.
• Assist members with filling out paperwork through electronic kiosks as needed.
• Request medical records and upload documentation to EMR.
• Field questions from members and their adult children.
• Manage phone line, take messages, and relay details to the right person.
• Assist with center events as needed.
