Qualifications

• Master's degree in Social Work, Counseling, Human Services, or a related field.
• Minimum 3-5 years of professional experience in care coordination or similar roles.
• Minimum 2 years of supervisory/leadership experience.
• DC license (LPC or LICSW).
• Demonstrated expertise in high-fidelity wraparound services or similar models.
• Strong knowledge of behavioral health, mental health, or substance use services.
• Excellent written and verbal communication skills.
• Strong analytical and problem-solving skills.
• Commitment to trauma-informed care practices and cultural competency.
• Valid driver’s license and reliable transportation.

Responsibilities

• Supervise, mentor, and develop a team of care coordinators with regular feedback and growth opportunities.
• Oversee delivery of high-fidelity wraparound services aligned with evidence-based practices.
• Conduct comprehensive clinical assessments and develop individualized care plans.
• Coordinate communication among multidisciplinary teams and families.
• Monitor caseload management and service utilization for efficiency.
• Analyze program data and outcomes to identify improvement opportunities.
• Maintain accurate documentation in compliance with regulations and policies.
• Respond to crisis situations with clinical judgment and guidance to staff.
• Participate in treatment planning meetings and interdisciplinary discussions.
• Build relationships with community partners to enhance service access.
• Ensure compliance with regulations, licensing, and organizational policies.
• Support onboarding and training of new staff.
• Advocate for participants’ needs while maintaining professional boundaries.
• Contribute to program evaluation and quality assurance.
