Job summary

Integrated Services for Behavioral Health is seeking a Care Coordination Supervisor to lead our wraparound care coordination team in Ohio. You will oversee daily operations, provide staff training in home and community settings, and ensure adherence to high-fidelity wraparound models for diverse families.

The role emphasizes collaboration with staff, clients, and community partners, maintaining Medicaid standards, and evaluating performance.

Qualifications

• Bachelor's or master's degree in a related field
• LSW/LPC license is required
• At least 3 years of experience providing coordinated care/care coordination services
• Previous supervisory experience preferred
• Experience in behavioral health field preferred, with multi-system youth or autism/developmental disabilities advantageous

Responsibilities

• Supervise and oversee care coordination team members through meetings, supervision, and regular communications.
• Provide training for direct care coordinators in home and community settings.
• Deliver routine training and support to staff through conferences and case problem analysis to improve skills.
• Promote a culture of teamwork and recognition among staff.
• Ensure documentation meets Medicaid standards and fidelity of wraparound services.
• Coordinate on-call rotation and participate in fidelity review and system of care implementation.
