Job summary

Catholic Community Services of Western Washington is seeking a Care Coordinator to deliver intensive, community-based therapeutic services to children, youth and families with serious mental health needs. You will build a Wraparound team to increase safety, stabilize placements and support identified needs.

The role requires a BA degree, 1 year of relevant experience, and the ability to work in home and community settings. Expect on-call duties and collaboration with a multidisciplinary team.

Qualifications

• BA degree is required.
• 1 year direct experience with youth and/or families in a structured professional setting.
• Ability to work in home and community settings.
• Must have reliable transportation and valid driver’s license.
• Must provide driving record and be able to obtain WA State Agency Affiliate Counselor Certification.

Responsibilities

• Provide Wraparound services and team-based care planning.
• Engage families with compassion, respect and support.
• Assess safety and stabilization needs and coordinate crisis response.
• Create and document individualized care plans with the family team.
• Coordinate with clinical supervisors and other professionals as needed.
• Participate in on-call rotation and crisis management.
