Catholic Community Services of Western Washington is seeking a Care Coordinator to deliver intensive, community-based therapeutic services for children, youth and families with serious mental health needs.

You will use Wraparound principles to build a team of supports to increase safety, stabilize placements, and support families in meeting identified needs. The role includes crisis management and collaboration with a multidisciplinary team.

Competenze

• BA degree required.
• 1-year direct experience with youth and/or families in a professional setting.
• Ability to work in home and community settings.
• Must have reliable transportation, valid driver’s license and automobile insurance.
• Must provide proof of acceptable driving record.
• Ability and willingness to obtain WA State Agency Affiliate Counselor Certification.
• Must work both independently and as part of a team.

Mansioni

• Assist in crisis management and on-call rotation.
• Provide intensive, community-based therapeutic services using Wraparound with families.
• Coordinate with a multidisciplinary team and natural supports.
• Prepare and maintain required documentation within established timeframes.
