Catholic Community Services is seeking a Care Coordinator in Washington to provide intensive, community-based therapeutic services using Wraparound for children, youth and families with mental health needs. The role emphasizes collaboration with a team, crisis management, and developing individualized care plans.

Minimum requirements include a BA degree, 1 year of relevant experience, and ability to work in home/community settings; drivers license and transportation are required.

Competenze

• BA degree required.
• 1 year direct experience with youth or families in a professional setting.
• Ability to work in home and community settings.
• Reliable transportation, valid driver's license, and auto insurance.
• Must provide acceptable driving record.
• Willingness to obtain WA State Agency Affiliate Counselor Certification.
• Ability to work independently and in a team.
• Support the mission and values of Catholic Community Services.
