The role requires a Master's in a related field, 3–5 years in care coordination with at least 2 in supervision, and a DC license (LPC or LICSW). A valid driver’s license and excellent communication are essential.

Qualifications

• Master's degree in a relevant field and DC licensure (LPC or LICSW).
• 3–5 years of care coordination experience, with at least 2 years in supervision/leadership.
• DC license (LPC or LICSW) and valid driver's license.
• Strong knowledge of high-fidelity wraparound or similar intensive care coordination models.
• Excellent written and verbal communication with data interpretation skills.
• Commitment to trauma-informed care and cultural competency.

Responsibilities

• Supervise, mentor, and develop a team of care coordinators with feedback and growth opportunities.
• Oversee delivery of high-fidelity wraparound services following clinical standards.
• Conduct clinical assessments and create individualized care plans.
• Coordinate communication among multidisciplinary teams, providers, and families.
• Monitor caseloads, service utilization, and program efficiency.
• Analyze program data and outcomes to identify improvements.
• Maintain accurate documentation in compliance with regulations.
• Respond to crises with clinical judgment and guidance.
• Participate in treatment planning meetings and case conferences.
• Build relationships with community partners and stakeholders.
• Ensure regulatory and organizational policy compliance.
• Support onboarding and training of new staff.
• Advocate for participants' rights and needs, maintaining professional boundaries.
• Contribute to program evaluation and quality assurance initiatives.
