Job summary

Better Morning, Inc. is seeking a Care Coordinator to provide high-quality, strength-based, family-centered wraparound support.

You will work with clients, families, and service providers to create and manage individualized care plans, identify needs, access services, monitor progress, and advocate for clients within a collaborative framework. The role requires a Master’s- or Bachelor’s-level degree and at least 2 years of experience in case management or social services.

Qualifications

• Bachelor’s degree in social work, psychology, counseling, or related field.
• 2+ years of case management or care coordination experience.
• Experience with wraparound services is a plus.

Responsibilities

• Engage with clients and families to establish trust and gather information.
• Conduct needs assessments to identify strengths, resources, and barriers affecting the family.
• Develop culturally responsive strategies to address the individual needs of each family.
• Carry a caseload of 10 families.
• Develop personalized care plans addressing mental health, education, physical health, and social needs.
• Coordinate services across providers (mental health, medical, educational, social services).
• Collaborate with wraparound team members to implement care plans.
• Advocate for clients and families within different service systems to ensure support.
• Facilitate family meetings and care team meetings to review progress and adjust goals.
• Track progress and adjust care plans; maintain case documentation.
