Job summary

Integrated Services for Behavioral Health in Ohio seeks a Care Coordination Supervisor to lead the wraparound care coordination team, ensuring high fidelity practices and collaboration with families and community partners.

You will supervise staff, provide training, monitor documentation for Medicaid standards, and coordinate on‑call coverage across Region H; bachelor’s/master’s degree and LSW/LPC license required.

Qualifications

• Bachelor’s or Master’s degree in social work, education, counseling, psychology, or related field preferred.
• LSW/LPC license is required.
• Three years of experience providing coordinated supportive services/care coordination.
• Previous supervisory experience preferred.
• Experience in behavioral health, with multi-system youth or autism/developmental disabilities preferred.
• Knowledge of regulations for behavioral health and related services.

Responsibilities

• Supervise and oversee care coordination team activities and supervision.
• Provide training for direct care coordinators in home and community settings.
• Support staff through conferences, case analysis, and skill improvement.
• Promote team recognition and collaboration.
• Ensure documentation meets Medicaid standards and high fidelity wraparound practices.
• Coordinate on‑call rotation and fidelity review.
