Job summary

ArchWell Health seeks a Care Navigator to create a positive, home-like experience for senior members. You will welcome patients, help with check-in, and guide them through the center journey while coordinating care with a multidisciplinary team.

The ideal candidate demonstrates outstanding customer service, familiarity with EMR systems, and strong communication skills. Spanish language skills are a plus in our diverse communities served.

Qualifications

• Strong customer-service orientation and welcoming demeanor.
• Experience in clinical or healthcare setting preferred.
• Fluency in Spanish or multilingual abilities beneficial where needed.

Responsibilities

• Greet members on arrival and guide them through check-in process.
• Update member information in EMR and coordinate referrals.
• Schedule appointments and coordinate multi-disciplinary care team referrals.
• Assist members with paperwork and using electronic kiosks.
• Field questions from members and families and route to appropriate staff.
