Job summary

ArchWell Health in the United States, Florida, Fort Myers, seeks a Care Navigator to welcome members and families, guide them through the visit, and ensure a positive experience. The role focuses on customer service, care coordination, and efficient handling of paperwork and referrals.

The Care Navigator will update EMR records, coordinate appointments with a multidisciplinary team, and support inquiries from members and adult children while maintaining professionalism in a fast-paced clinic

Qualifications

• Associates degree preferred, or equivalent experience.
• Minimum of one year in a clinical setting or similar.
• Experience working with EMR systems is a plus.

Responsibilities

• Greet members and help them feel safe and at home during check-in.
• Update member information in the EMR system.
• Schedule appointments and coordinate referrals for a multi-disciplinary care team.
• Assist members with filling out paperwork via electronic kiosks.
• Request medical records and upload to EMR.
• Field questions from members and their families.
• Manage the phone line and relay details to the right team.
• Assist with center events as needed.
