Job summary

ArchWell Health in Jacksonville is seeking a Care Navigator to ensure a welcoming member experience from arrival through departure. You will guide members through check-in, update EMR records, coordinate referrals, and assist with paperwork and inquiries.

Qualifications

• Associates degree preferred; or equivalent experience.
• Minimum of one year of work experience in a clinical setting or similar.
• Experience working with EMR systems is a plus.

Responsibilities

• Welcome members and guide them to a smooth check-in.
• Update member information in the EMR system.
• Schedule appointments and coordinate referrals for a multidisciplinary care team.
• Assist members with filling out paperwork through electronic kiosks.
• Request medical records and upload to the EMR system.
• Field questions from members and their families.
• Manage phone line, take detailed messages and relay details to the appropriate person.
• Assist with center events as needed.
