ArchWell Health in Greensboro, NC is seeking a Care Navigator to greet members and guide them through check-in, scheduling, and EMR processing. You will coordinate referrals for a multidisciplinary team, answer questions, and support center events while delivering compassionate, patient-centered service to seniors.

Required: Associates degree preferred or equivalent experience, at least one year in a clinical setting, and basic knowledge of EMR systems.

Qualifications

• Associates degree preferred or equivalent experience.
• Minimum of one year of work experience in a clinical setting.
• Experience working with EMR systems is a plus.

Responsabilités

• Welcome members and help them check in, making them feel at home.
• Update member information in the electronic medical records system.
• Schedule appointments and coordinate referrals for a multi-disciplinary care team.
• Assist members with filling out paperwork via electronic kiosks.
• Request medical records and upload to EMR.
• Field questions from members and their families.
• Manage center phone line and relay messages to the right team.
• Assist with center events as needed.
