Job summary

ArchWell Health is seeking a Care Navigator to provide a positive, welcoming experience for our members and their families at our Port Charlotte centers. You will guide members from arrival to departure, ensuring needs are met with a high level of service.

The ideal candidate will have a strong customer-service orientation, be passionate about senior care, and able to work effectively with diverse populations, including families and staff. Fluency in Spanish is a plus.

Qualifications

• Associates degree preferred, or equivalent experience.
• Minimum of one year of work experience in a clinical setting.
• Experience working with EMR systems is a plus.

Responsibilities

• Welcome members and guide them through check-in.
• Update member information in EMR.
• Schedule appointments and referrals for care team.
• Assist with paperwork via electronic kiosks.
• Request and upload medical records to EMR.
• Answer questions from members and families.
• Manage front desk phone line and messages.
• Support center events as needed.
