Job summary

Hartford HealthCare at Home is seeking a QAPI Specialist to advance clinical documentation and outcomes using a data-driven approach. You will compile trends, report quality data across patient care, safety, risk, infection control, outcomes and satisfaction, and lead performance improvement plans.

Responsibilities include audits, development of improvement interventions, regulatory compliance, and collaboration with clinical staff. Travel to branches within the region may be required.

Qualifications

• 3 years of certified home health experience required.
• Value Based Purchasing, PDGM, CAHPS-preferred.
• Licensure, Certification, Registration.
• CT Licensure in field of specialty.

Responsibilities

• Develop and implement performance improvement plans and actions.
• Audit clinical records and provide feedback to staff.
• Coordinate data analysis and reporting for quality metrics.
• Ensure compliance with federal and state regulations and payor requirements.
• Travel to branches (regions) as needed.
