Job summary

Ideal candidates have 3 years of certified home health experience, CT licensure in the specialty, and strong communication skills. Travel to branches across the United States may be required as part of the role.

Qualifications

• 3 years of certified home health experience
• Knowledge of Value Based Purchasing, PDGM, CAHPS-preferred
• CT Licensure in field of specialty
• Licensure, Certification, Registration
• Excellent written and verbal communication skills
• Willingness to travel within branches and US travel as needed

Responsibilities

• Assist with development of Performance Improvement Plans
• Compile trends and reports quality data in patient care, safety, risk management, infection control, outcomes, and customer satisfaction
• Lead audits and clinical record reviews to support quality improvement
• Develop interventions to impact quality scores and participate in PI teams
• Ensure compliance with federal and state regulations and organization policy
• Facilitate evidence-based guidelines to reduce variation in clinical practice and optimize outcomes
