Job summary

Hartford HealthCare at Home seeks a QAPI Specialist to enhance clinical documentation and outcomes using data-driven methods. You will compile quality data across patient care, safety, risk management, infection control, outcomes and satisfaction, and lead performance improvement plans based on audit results.

You will review records, provide staff education, and promote evidence-based guidelines to minimize practice variation while ensuring regulatory compliance and timely reporting to payors

Qualifications

• 3 years of certified home health experience required.
• Value Based Purchasing, PDGM, CAHPS-preferred.
• CT Licensure in field of specialty.

Responsibilities

• Compile trends and reports quality data on patient care, safety, risk management, infection control, outcomes and satisfaction.
• Lead development of Performance Improvement Plans from audit results.
• Conduct clinical record audits and provide staff education.
• Promote evidence-based guidelines to reduce variation in clinical practice.
• Ensure regulatory compliance and timely data submissions to payors and regulators.
• Travel to branches as needed.
