Job summary

Qualifications

• 3 years of certified home health experience required.
• Experience with Value Based Purchasing, PDGM, CAHPS-preferred.
• CT Licensure in field of specialty; Licensure, Certification, Registration required.

Responsibilities

• Assist with improving clinical documentation and outcomes using data-driven approaches.
• Compile trends and reports on quality data across patient care, safety, risk, infection control, outcomes and customer satisfaction.
• Lead development of Performance Improvement Plans based on audit results.
• Develop and implement process improvement plans; conduct record reviews and data analysis.
• Coordinate with agency to ensure compliance with federal and state regulations and submit data to payors/regulators.
• Travel to branches as needed to support quality initiatives.
