Job summary

Hartford HealthCare at Home is seeking a Quality Assurance & Performance Improvement Specialist to lead data-driven efforts to improve clinical documentation, patient outcomes, and safety across the agency.

You will compile trends, audits, and reports on patient care, safety, infection control, OASIS, and satisfaction, and develop performance improvement plans. Requires 3 years of home health experience, CT licensure, and willingness to travel to branches.

Qualifications

• 3 years of certified home health experience.
• Knowledge of Value Based Purchasing, PDGM, CAHPS preferred.
• CT Licensure in field of specialty.

Responsibilities

• Develop and implement clinical quality improvement action plans.
• Perform clinical record audits and provide educational feedback to staff.
• Review OASIS assessments for completeness and compliance.
• Ensure appropriate ICD-10 coding and sequencing.
• Facilitate PI teams using quality improvement methodology.
• Assist with data analysis and reporting for regulatory bodies.
