Job summary

Hartford HealthCare at Home is seeking a QAPI Specialist to drive data-driven improvements in clinical documentation, patient safety, and outcomes. You will collect trends, generate reports, and lead performance improvement plans in partnership with clinical teams.

Requirements include CT licensure in the field, 3 years of certified home health experience, and strong analytical, educational, and communication skills. Some travel to branches nationwide is expected.

Qualifications

• Experience in data analysis and quality reporting.
• Ability to design performance measures and track outcomes.
• Experience providing education and leading PI teams.
• Strong written and verbal communication skills.
• Knowledge of federal/state regulatory requirements.

Responsibilities

• Participates in development and evaluation of clinical quality improvement action plans.
• Leads data collection, audits, and reporting to drive improvement.
• Reviews OASIS, coding, and compliance with regulations.
• Advances performance improvement plans with clinical staff.
• Facilitates PI teams using quality improvement methodology.
• Travels to branches to support enterprise quality initiatives.
